Processing Fees Are Paid By NIHCA.
 The club will provide Apex a CSV file once a month and Apex will provide you with the specific format. Apex will process the file and release funds from the HMO within 30 days. Apex will send remittance reports and a check on the 1st and 16th of every month. The club will adjust the individual members billing account each time a payment is received.
